Capsulitis / Synovitis

Synovitis or Capsulitis is an inflammation of the joint and is usually accompanied by an increased production of synovial fluid that can cause the joint to swell.

What causes Capsulitis/Synovitis

Our joints are held in place and supported by our ligaments. Their external covering of the joint is known as the joint capsule.  The Synovial membrane is the name given to the inner layer of the joint capsule.  This Synovial membrane produces the oily substance called synovial fluid that lubricates the joint. Synovitis or Capsulitis is an inflammation of the joint and is usually accompanied by an increased production of synovial fluid that can cause the joint to swell.

  • Overloading from an associated deformity
  • Poor foot function
  • Unsuitable footwear [high heels etc.]
  • Rheumatoid arthritis can lead to inflammation within the joints.

Our 4th and 5th metatarsals, the long bones that run from the small and second smallest toes toward the ankle are the most mobile of the metatarsal bones. As we walk and run these bones maneuver to allow us negotiate uneven surfaces.  Should these metatarsals not load correctly in the gait cycle then overloading and inflammation of the joint can occur.

Women are more likely to suffer from Capsulitis/Synovitis as they tend to wear narrower/tighter shoes. Shoes that are too small can aggravate the problem.

Commonly when people experience pain and discomfort in these joints they fail to bring it to the attention of a Podiatrist, hoping the pain will cease. However, typically the pain increases.  This results in the toes bending and becoming prominent.  Over time the pain may settle, however, the toes will not lie level with the big toe but instead will sit up, causing an issue in the shoe, this is a result of the long-term inflammation of the joint and damage to the joint capsule, resulting in a rupture. In such instances, the toe is destabilised causing the toe to become deformed and prominent.  Here the pain can settle, however, the toe will cause discomfort in shoes, this is much harder to treat.

Can I reduce the pain caused by Capsulitis / Synovitis?

A clinical examination and patient history can allow for diagnosis.  Various forms of scanning methods including X-ray, Ultrasound and MRI can reveal damage to the joint capsule. To reduce the pain:  

  • Wear good fitting suitable shoes - avoid high heels 
  • Use protective pads to cushion the affected area
  • See a podiatrist

With the correct attention the contributing factors can be treated reducing pain and discomfort.  If the problem is not treated the problem will continue to get worse resulting in ruption and toe deformity. If the condition persists surgery may be advisable.
